Post-Admission Task-List for Newly Admitted AT Majors

The following items must be completed once you are admitted to the Athletic Training major (i.e. for rising sophomores accepted to AT).  They do not apply to incoming freshman.

Before the end of Spring Quarter:

  1. Spring Quarter Academics - complete any spring quarter pre-requisite AT courses with a grade of "C-" or better. Earning a grade below "C-" results in cancelation of your offer of admission.

  2. Acceptance and Fee: If you are a currently enrolled OSU student, you do not have any acceptance fee.  You can accept your position in the program by checking your application status online. If you are not a currently admitted OSU student, you will have an acceptance fee to pay and you must sign and return the acceptance form and fee by the deadline listed in the letter.  You can not register for the AT courses until you do.

  3. Khaki Pants & Shorts - You must purchase the official khaki pants and shorts required for your Athletic Traning Clinical Uniform. Substitutes will not be accepted. You will need these for clinicals in August, but they sell out quickly so purchase them now.  If you wait, they WILL be sold-out and you will not be prepared for August clinicals. The official uniform pants and shorts are available from JCrew at a group purchase discount.  See instructions for ordering.

    DEADLINE: Wear your khaki shorts to your clinical orientation in August.

  4. Background Check - Pass a background check conducted through the OSU Medical Center Security Department.  Becca Roby has contacted you by e-mail with further instructions (note these are the 2008 instructions... ignore the dates).  The OSUMC Security department is located between the Ross Heart Hospital and Rhodes Hall (S-127 Rhodes Hall - across from the Hospital Cafeteria Vending Area).  Their hours are 7:00 - 10:30 am and 1:00 - 4:15 pm M-Th and Friday by appoinment.  You should be able to have your background check performed starting on June 4th. Be sure to take your drivers license and social security card with you. See this MAP for the location [look for the police badge icon on the map].

    DEADLINE: Please complete your background check BEFORE the end of finals week during the spring quarter (even though the date on the instructions is later than this... we need these all processed before you start your clinicals in August).

  5. OSUMC Identification Badge - When you have your Background Check, also have your photo taken for an OSUMC ID Badge (office is next to OSUMC Security Dept).  The security office will send your badge to Becca Roby in our office when it is complete.

  6. Summer Housing - University Housing (dorms) are not open in August & September during the time when you must be here for clinicals.  You will need to arrange for housing during this time at your own expense.  Most students room with other program students, family, friends, etc for this 6 - 8 week period.  The AT Student Club hosts an event known as the "Meet & Greet" for incoming students toward the end of spring quarter [June 2, 2009, 5:00 pm, WHAC Atrium].  This is an excellent opportunity to network with your classmates and with returning students for housing opportunities.  The AT Division does not arrange for your summer housing.

Over the summer before you report for Clinicals in August:

  1. Technical Standards and Physical Exam: Read and understand the program's Technical Standards for Athletic Training Students and submit the Certification Statement. The Technical Standards describe the essential functional requirements for enrollment in the program.  All students must be able to meet these standards (with or without accommodation) in order to enroll in Athletic Training courses or clinicals. The Certification Statement requires the student's signature AND a physical exam verifying ability to meet the Technical Standards.  The physical exam should be performed by your family physician, physician assistant, or Certified Nurse Practitioner who must also sign the form. It often takes several weeks to get an appointment for a physical exam, so start on this in June in order to meet the deadline. Failure to have this on-file will prevent you from beginning clinicals in August. Be sure to also send a copy of your physical exam with your immunization form (see below)

    DEADLINE:
    Technical Standards form must be received in the AT Office no later than Friday July 17th [mail to: Athletic Training Division, 453 W. 10th Ave, Columbus OH 43210, OR fax: 614-292-0210]. Failure to have this on-file will prevent you from beginning clinicals in August.  Send your physical along with your immunzation records to student health.

  2. Immunizations and Tuberculosis Test: Your immunizations must be current and you must have a tuberculosis test (one step PPD).  Submit your vaccination and test records to THE WILCE STUDENT HEALTH CENTER (not the AT office). Required Vaccinations include the university's standard list + Hepatitis B.  The immunization and Health Screen form shows the list of immunizations you need and can be used to document your immunizations.  If the form is used, make sure it is signed by the physician who can verify the record is accurate.  You should send BOTH your immunization information AND a copy of your physical exam to student health. You may begin the HepB vaccinations over the summer and submit documentation while you are in process.  When complete you will also need to send the verification blood test or suface antibody test. You may complete vaccinations and the TB test with your family physician or at the Student Health Center. Any costs for vaccines, Hep B antibody test or the TB test are the responsibility of the student. 

    DEADLINE
    : submit to Student Health along with a copy of your physical exam no later than Friday July 17th. Failure to have these on-file will prevent you from beginning clinicals in August.

  3. First-Aid, AED and CPR Certification: You must be certified in Professional Rescuer / Healthcare Provider CPR and in First-Aid at all times in clinical rotations.  We only accept CPR certifications that involve in-person demonstration of skills (no CPR certifications that are completed online).  CPR must be 2 rescuer for adults, children, and infants and must include bag valve masks. Regular Adult CPR does not meet the requirement.

    DEADLINE
    : Documentation of certification that will last through the entire academic year must be received in the AT Office no later than the Friday July 17th. Documentation can be photocopies of cards (both sides) or a letter/certificate from the instructor documenting the course and the dates.  There are several common ways to obtain this certification:
    1. American Red Cross:
      • Standard First-Aid (~$45) + CPR/AED for the Professional Rescuer (~$80)
    2. American Heart Association:
      • Heartsaver First-Aid + BLS Healthcare Provider
    3. Ohio State University:
      • Allied Med 200 + EDUPAES 102  (Note: the EDUPAES 122 CPR course is not acceptable)
    4. National Safety Council:
      • NSC does not offer a CPR course that meets our needs (need 2 person CPR)

  4. Autumn Quarter Course Scheduling - AFTER you accept your position in the program (by submitting the form and fee [if applicable]), your major will be officially changed to ATH TRNG. Once this happens, you will need to change your autumn quarter course schedule.  You should take:
    1. Anatomy 200 - you are administratively added to this... don't drop it unless you already have credit for the course because you can't get back in!
    2. ATH TRNG 350 - schedule this for autumn, but you actually complete it in August and September in conjunction with your summer clinical rotations (two-a-days).
    3. ATH TRNG 420 - your first injury evaluation course (lower extremity)
    4. ATH TRNG 430 - your first Experience in Athletic Training course.
    5. Most students also complete one elective or GEC course during the autumn.

  5. AT Student Handbook - read and be familiar with ALL expectations, policies and procedure outlined in the AT Student Handbook [2009 handbook already emailed to you].  The handbook explains the program, the academic and clinical expectations and requirements to remain in good standing.  Ignorance of policies and requirements is not a valid excuse for violating them.

    DEADLINE
    : Read the entire handbook before reporting for the Clinical Orientation in August.

  6. Summer Clinical Site Transportation - off-campus clinical rotations are a required part of the summer experience.
    • You will complete as many as three differenent off-campus rotations, some may be as far as Delaware OH.
    • Transportation to and from off-campus sites is solely the responsibility of the student.  The AT Division does not arrange or provide your transportation.
    • Students often work together to carpool to these sites.  If you ride with someone else, you are expected to either reciprocate by helping to pay for fuel or by driving others in your vehicle on other days.
    • Parking Passes - if you need a campus parking pass, please inform Dr. Merrick at the clinical orientation meeting so that he can arrange for permission for a parking pass for you.

  7. Clinical Orientation Meeting - All students must attend the Clinical Orientation on Monday August 3rd at 9:00 am at the Woody Hayes Athletic Center atrium (same place as the meet & greet).
    1. You must arrange for your summer [Aug - Sept] housing to start on or before this date.  We do not provide your housing.
    2. At this meeting you will go through clinical orientation, receive your initial clinical instructor assignment and be issued your OSU Athletic Training uniform (you must have your khaki pants and short by this time... see above).
    3. Your clinicals begin on this date, so your summer employment must end before this.  It is not possible to have a job during the summer clinical rotation period.
    4. Make sure you have completed /we have received the following items before the clinical orientation meeting.  If you are missing any, you will prevented from beginning clinicals until they are received:
      • Khaki pants and shorts (buy them before spring qtr ends!)
      • Background check & OSUMC ID photo
      • Technical Standards Certification / Physical Exam
      • Immunizations & TB Test
      • First-Aid and CPR Certification

After you arrive for Clinicals in August:

  1. Bloodborne Pathogen - You will be enrolled in an Infectious Disease Control module on Carmen.  Becca Roby will let you know when this is ready to complete.

  2. HIPAA Medical Privacy Training - You will be enrolled in a HIPAA training module on Carmen.  Becca Roby will let you know when this is ready to complete.
