1. View the selections below and choose the appropriate size and style.
   
   **Length of shorts must be below finger-tips when standing with your arms at your sides.  You will be sent home if you do not meet this requirement.**



2. An electronic order form will be sent to your osu e-mail account upon verbal acceptance of admission to the program. Replacement forms can be obtained by contacting Rebecca Roby in the AT office. Rebecca.roby@osumc.edu or 614-292-3702.
   
   
3. Fill out the order form and e-mail or fax it to Amanda Brooks at amanda.brooks@jcrew.com or 434-385-5750.  Note: If submitting the form by E-mail, please put "Ohio State Athletic Traing" as the subject. If faxing, please note "Ohio State Athletic Training" on the cover sheet.
