1.) Graduate School Application (On-line application)
2.) A non refundable forty dollar admission fee.  Fifty dollars for international applicants. 
3.) One official copy of transcripts or record of marks for each undergraduate school attended, except      for The Ohio State University. For international students, include English translation of each and any additional foreign documents.
4.) Official GRE scores.  The institutional code is 1592.
5.) Official TOEFL test scores for international students.  The institutional code is 1592.

2.) Three letters of recommendation, including the graduate reference forms.

Application Review
Each application is reviewed by three faculty members of the SAMP Graduate Studies Committee; the Graduate Studies Chairperson, coordinator of the Area of Emphasis to which the applicant is applying and one other member.  The Graduate Program Manager and the Graduate Studies Chairperson select the reviewing faculty members.  

Review of the application begins when all of the application materials have been received and proceeds to completion in a period of two weeks.  Applicants can check the status of their application at http://www.appstatus.osu.edu/.

Graduate Non Degree Status
Individuals who do not wish to apply to the program at this time, yet would like to take graduate course work within the program or Graduate School may apply for admission as a graduate non-degree student.  On verification of a baccalaureate degree from an accredited institution, graduate non degree students may register for graduate level courses.  To obtain an application, please visit http://www-afa.adm.ohio-state.edu/grad/nondegree.html.

If a graduate non degree student later applies and is admitted to a graduate degree program, no more than ten hours of graduate credit accumulated as a non degree student can be counted toward the graduate degree.  The appropriate form must be completed by the student and graduate program for this to occur.
